Family child care licensing packet
For your convenience, we've gathered together critical documents and forms that must be reviewed and completed annually for your license as a family child care provider.
Completed forms must be provided to your regional licensing specialist at your annual inspection visit.
Please do not mail anything.
Applications and forms:
- Enrollment Form (PDF)
- Required Reporting Procedures and Household Roster (PDF) - Ensure every household member and individual who is a sub, caregiver, or helper is listed
- Worker's Compensation Form (PDF)
- Licensed Space Form (PDF)
- SFCC Program Plan (PDF) ONLY if operating somewhere that is not your primary residence
- Application (PDF) - ONLY if you are requesting a change to your license capacity
- No fee is required with the annual inspection.

Background studies
In light of changes to state and federal laws, the Minnesota Department of Children Youth and Families has finalized guidance for family child care providers regarding who needs a background study. You may access a Background Study Worksheet (PDF) and find additional information on the DCYF website.
If you need to complete a background study, please contact your county's regional licensing specialist.
