Family child care licensing packet
For your convenience, we've gathered together critical documents and forms that must be reviewed and completed to renew your license as a family child care provider.
Applications and forms:
- Family Child Care - Family Systems Family Child Care Application (PDF)
- Complete ONLY if you are operating your program outside of your residence: Program Plan for Special Applications (PDF)
- Enrollment Form (PDF)
- Licensed Space Form (PDF)
- Required Reporting Procedures and Household Roster (PDF)
- Certificate of Compliance Minnesota Workers' Compensation Law (PDF)
- Annual Reporting Caregiver Information and Sub Use Form (PDF) only applicable if you use helpers and other caregivers in your program
- A list of days you are closed in the two months prior to your license expiration
- Commissioner Access Letter (PDF)
Completed forms must be provided to Regional Licensing Specialist at your Annual Inspection Visit.
Please do not mail anything back.
Background studies
In light of changes to state and federal laws, the Minnesota Department of Human Services has finalized guidance for family child care providers regarding who needs a background study. You may access a Background Study Worksheet (PDF) and find additional information on the DHS website.
If you need to complete a background study, please contact your county's Regional Licensing Specialist.
